Finally approved at day 5. Package was poorly packed when arrived for a 3 lb piece of metal. It was packaged in a padded priority mail bag and thrown in a plastic mailer bag. Metal tab was bent and powder coating was chipped. Seemed to be a problem with hitch alignment bolt pattern too.
